Safer Discharge for Patients Workshop

Overview

This free training day supports you and your organisation to develop the skills, knowledge and confidence to improve the quality of hospital discharges and transfers of care. The focus is on improving the patient experience of hospital discharge by improving the quality of discharge planning. In turn this should lead to a reduction in emergency re-admissions, a reduction in delayed transfers of care and patients returning to optimal independence in the community. The day includes an interactive theatre session which allows participants to learn by doing, challenge their own thinking by identifying with other perspectives and explore new solutions and strategies to replace old behaviours.

Through the use of case-based discussions and practice-based learning,  participants will develop knowledge and skills in the following areas:

  •     Initial assessment and risk assessment
  •     Key roles and responsibilities
  •     Keeping the patient at the heart of communication
  •    Complex discharge

 

Additional Information

The Safer Discharge for Patients Workshop welcomes any doctors, nurses, students, HCA’s therapists, community and social care sectors involved in the planning process for patients to attend. The programme begins at 08:50 and ends at 17:00. Places are free for acute and non-acute sector healthcare workers however a £100 non-attendance fee is applicable if you do not attend or cancel without providing two week's notice.
